With the help of the anamnesis questionnaires you can ask your members about their goals and previous experiences before creating a training plan or specifically about their current condition during training plan appointments. You can formulate individual questions and create several forms. Please note that in order to create anamnesis sheets, anamnesis questions has to be created first.
Create anamnesis sheet
After you have prepared various anamnesis questions, you can now combine them into an anamnesis sheet under Training World / Assessment / Questionnaire. Click on Create Create anamnesis form in the upper right corner. First, define a name for the questionnaire and the target group for the survey. After saving this framework data, the questionnaire will now appear in the list of questionnaires that have already been created.
Then add categories via the three-dot menu in the row of the sheet by clicking on Add category to sort the created questions thematically. They provide the framework for adding medical history questions. If you create more than one category, you can then use Sort order of categories to specify the order in which the categories and, accordingly, the questions of the case history sheet are queried.
Questions can be added via the three-point menu of the created category. To do this, click on Edit questionnaire. All active anamnesis questions are available for selection and can be selected using the drag & drop function or the arrow keys for the sheet. The order of the questions can also be changed via drag-and-drop. The medical history sheet is saved and activated via Save. This means that it can be selected for your anamnesis appointments under the member profile under Training / Assessment / New Assessment.
Edit anamnesis sheet
If you want to make changes to the medical history sheet, use the Edit assesment form function in the three-dot menu. Now you can specify the name and the target group of the form again. If you no longer wish to use the form for conducting medical history interviews with members, click Deactivate assesment form. The sheet will then no longer be available via the member profile under Training / Assessment / New Assessmen. If you want to delete the anamnesis sheet irrevocably, click Remove assesment form.
